  M/s ... company Name ......  It has been observed on Date......,at ...... address of site that men-power working at site are not using PPE and violating safety rules and regulations during execution of work that could lead to major or fatal injuries to them. Therefore you are suggested to ensure the safety of men-power at site.  Note - Some issues are also found listed below. 1 - No Work permit. 2- 15 No of mdpe joints dia 180 mm done without witnesses.  NOTE - Pictures attached.

 Dear sir, As per joint site visit at .... place .........address ......as on date................. along with Mr.....name.. company site........& ..... contractor site name.....we observed that the laying of proposed dia ...(20mm......32mm ..63mm...)....Gas pipeline is not possible by open tranch/ mouiling method due to 200 mm thick pcc road. As the said work to be done on priority due to construction work of (building name).....to avoid any kind of damage of igl gas pipeline. Hance, In view of the urgency of matter and site conditions it is recommended to lay the dia .....gas pipeline by HDD method. Regards Name Inspection Engineer
