How to write a warning mail regarding violation of safety.

 It has been observed on Date......,at ...... address of site that men-power working at site are not using PPE and violating safety rules and regulations during execution of work that could lead to major or fatal injuries to them.
Therefore you are suggested to ensure the safety of men-power at site. 

Note - Some issues are also found listed below.
1 - No Work permit.
2- 15 No of mdpe joints dia 180 mm done without witnesses.
