Dear M/s......

With reference to the above given matter, kindly ensure proper safety measures which has to be taken at work site. As during (work name )....GI installation and laying of mdpe lines following items and PPE,s should we checked before implementing any kind of work.

1- Safety helmet and safety shoes for the worker performing work at site.

2- Reflective jacket shuld be there, especially on traffic areas.

3- First Aid box.

4- warning board and proper barricading should be there at the work site.

5- Approved equipment's for ascending and descending provided at work at height.

6- Fire extinguisher should be there.

7- Strictly follow the permit issued by undersigned.

If any untoward incident happens, you will be held responsible for the same .

Now you are advised to aforesaid issues on utmost priority by maintaining proper communication with .....(company name).. representative else penal action against ignorance shall be taken as per terms and condition of contract.
